Yes, there's an app cache located within the app settings. Go to Apps > Next Episode > Storage and you'll see the following screen...

From there you can clear data and cache. As far as Android saving app data to the cloud, yes this is true, but there are also some limitations as to what gets saved, from a system-wide standpoint. You can simply just clear the app data, as mentioned above, and this will reset the app settings. You will still need to log back in at some point, and your settings will automatically resync from your next-episode online account settings. Just make sure all Notifications options are turned on for testing purposes. Once you pinpoint the problem, then you can fine tune those notifications settings as you prefer from within the app.
I'm not saying that the above steps will resolve your issue but you're welcome to explore those options. No need to uninstall and reinstall the app since you already tried that, and disabling the automatic app data restore process would be no different than clearing the app cache and data.
