Well lately I have been playing with aggressive expansion - as long as I have at least 4 happiness I go for a new city next to a new luxury resource (I stop expanding at around 5-6 cities). Spending a little time at "unhappy" has never stopped me . Dont forget to trade with the AI - if you got an abundance of one luxury resource they will go for an even swap if they have two of something as well. Oh and most of my happiness comes from religion later (because I usually build stonehenge as early as possible and get to cherry pick the features). So what kind of a build order do you guys use in your first city? Obviously it varies on circumstance a bit, but I usually go: scout-monument-worker-stonehenge-settler-pyramids-settler. Social policies sually I pick Tradition first just for the +3 culture, followed by liberty with an emphasis on getting the settler first. if there are no opponents nearby I might build the stonehenge before the first settler (especially on higher difficulties).
